What Is a Bunion?

A bunion is a bony prominence at the base of the big toe. It develops as the first metatarsal gradually shifts away from the second toe, while the big toe drifts toward the lesser toes. This changes the angle of the first metatarsophalangeal joint and creates the visible bump on the inside of the foot.

Bunions usually develop slowly over years. Without management, the deformity may progress — although the rate varies considerably between people.

Painful vs Non-Painful Bunions

Not every bunion is painful. Some people have a visible deformity for years with minimal symptoms, while others develop significant pain from a relatively small bunion. Pain does not always match the size or appearance of the deformity.

A bunion can still cause secondary problems even when it is not painful, including:

• Difficulty finding comfortable shoes
• Increased pressure under the ball of the foot
• Corns, callus or irritation between the toes
• Altered walking mechanics
• Pressure or crowding of the second toe
• Reduced big-toe joint movement

Painful bunions may cause tenderness over the bony prominence, redness or swelling from footwear friction, pain with walking or prolonged standing, stiffness in the big toe joint, or pain under the ball of the foot.

What Causes Bunions?

Bunions usually result from a combination of inherited foot structure, mechanical load and footwear factors. They are not simply caused by “bad shoes”, although footwear can aggravate symptoms and may contribute to progression in susceptible feet. Contributing factors may include:

• Family history or inherited foot shape
• Flat feet, excessive pronation or joint hypermobility
• Tight, narrow, pointed or high-heeled footwear
• Inflammatory arthritis, including rheumatoid arthritis
• Altered foot mechanics and muscle function
• Increased load from occupation, sport or prolonged standing

The Role of Foot Strengthening

The small intrinsic muscles of the foot help stabilise the arch and control the position of the first metatarsal during walking. When these muscles are weak or poorly conditioned, the foot may rely more heavily on passive structures and footwear to manage load.

Foot strengthening and rehabilitation may help improve foot function, reduce symptoms and support a more stable gait. However, it is important to be realistic: exercise can improve comfort and function, but it does not reliably straighten an established bunion or correct the underlying bony deformity.

Treatment & Management Options

Conservative care should usually be tried before surgery, particularly where symptoms are mild to moderate. The aim is to reduce pain, pressure and functional limitation, rather than to permanently reverse the deformity. Your podiatrist may recommend:

• Footwear modification — shoes with adequate length, a wide and deep toe box, cushioning and a lower heel can reduce pressure over the bunion and big-toe joint
• Padding and offloading — bunion pads, toe spacers or protective sleeves can reduce direct pressure and friction over the prominence
• Custom orthotics — podiatrist-prescribed orthotics may help address contributing foot mechanics, redistribute pressure and improve comfort
• Foot strengthening and rehabilitation — targeted exercises to improve intrinsic muscle strength and joint stability
• Activity and load management — modifying high-load activities during painful flare-ups
• Rocker sole and carbon fibre plate footwear — reduces the range of motion required at the big toe joint, significantly decreasing pain with each step

For patients with a biomechanical contributing factor, a comprehensive gait and biomechanical assessment may be recommended as part of your management plan.

Rocker Sole & Carbon Fibre Plate Footwear

For some people with painful bunions, footwear can make a substantial difference to day-to-day comfort. A rocker-sole shoe reduces the amount the big toe needs to bend during walking, which can decrease painful movement through the first metatarsophalangeal joint.

A carbon fibre plate adds stiffness through the forefoot and can further limit painful joint motion. These features may be particularly useful where the big toe joint is stiff, painful or irritated with every step.

Footwear advice should be individualised — the best option depends on your foot shape, bunion severity, joint mobility, activity level, work requirements and the shoes you are able to wear comfortably.

When Is Surgery Considered?

Surgery is generally considered when significant pain or deformity is affecting daily function and conservative treatment has not provided adequate relief. It is not routinely recommended for cosmetic correction alone.

Surgical options vary depending on the severity and location of the deformity, the mobility of the big toe joint and whether arthritis is present. Minimally invasive bunion surgery is an established option in appropriate cases.

Can bunions be fixed without surgery?

Do orthotics help bunions?
Orthotics can help manage symptoms by addressing underlying foot mechanics, redistributing pressure and improving comfort. Evidence for orthotics changing the bunion angle itself is mixed — they are best viewed as a symptom- and function-focused treatment rather than a cure.

Should I see a podiatrist if my bunion doesn’t hurt?
Yes. Even pain-free bunions can cause secondary problems including corns, altered gait, second-toe crowding and difficulty with footwear. Early assessment allows proactive management to slow progression before symptoms become significant.

What is the difference between a bunion and a corn?
A bunion is a structural deformity — a change in the alignment of the first metatarsal and big toe. A corn is a localised area of thickened skin that develops in response to pressure or friction. The two often co-exist, as the bony prominence of a bunion creates pressure against footwear.

Can children get bunions?
