How to refer: Referrals can be sent via fax or email. A standard GP Management Plan and Team Care Arrangement (TCA) is required. Please include relevant medical history and any specific concerns or goals for the patient's podiatry management.
We aim to see CCMP patients within a clinically appropriate timeframe and will contact you if we have any concerns following assessment.
What is a Chronic Condition Management Plan?
A Chronic Condition Management Plan, also known as a CCMP, is a Medicare initiative that allows patients with chronic or complex medical conditions to access a limited number of allied health consultations, including podiatry, with a partial Medicare rebate.
To access CCMP podiatry, you will need to speak to your GP, who will assess your eligibility and prepare the relevant paperwork — a GP Management Plan and Team Care Arrangement — that includes a referral to our clinic.
Who is eligible?
CCMP referrals for podiatry are available to patients with a chronic medical condition that has been, or is likely to be, present for six months or more and that requires ongoing management. Common conditions include diabetes, arthritis, neurological conditions and chronic musculoskeletal problems.

How does the Medicare rebate work?
Under a CCMP, eligible patients receive up to five allied health consultations per calendar year with a Medicare rebate applied to each visit.

What happens at your appointment?
Your CCMP podiatry consultation at Sydney Podiatry Co is comprehensive and unhurried. Your podiatrist will conduct a thorough assessment relevant to your chronic condition, develop or review your management plan, and provide treatment as appropriate. A full clinical report is sent to your referring GP following the first and last consultation on your plan.
